To update or change the name in an existing mutual fund folio, whether due to marriage, divorce or other valid circumstances—the investor must follow the procedure as given below. 


Required Documents:

Step 1: Update Name in KYC (if not already done)

Step 2: Submit Name Change Request (AMC-wise)

  1. Name Change Request Letter (AMC-wise)

  2. Self-attested PAN Card copy (with updated name)

  3. Self-attested Aadhaar Card copy (with updated name)

  4. Self-attested Bank Account Proof

    • Any one of the following:
      • Cancelled cheque with name printed
      • Passbook copy (front page + last 3 months transaction entries with Bank seal, Authorized person sign, designation and date)
      • Latest 3-month bank statement (With Bank seal, Authorized person sign, designation and date )
  5. Banker attestation for new signature - (Enclosed Annexure 1b)

Additional Documents Based on Reason for Name Change

1. Due to Marriage

  • Notarized copy of Marriage Certificate with unit holder self attestation
  • Self-attested Identity Proof with New Name
    • Acceptable: PAN, Voter ID, Driving License, Passport, PIO Card

2. Due to Divorce or Death of Spouse

  • Notarized copy of Divorce Deed or Death Certificate with unit holder self attestation 
  • Self-attested Identity Proof with New Name

3. Any Other Reason

  • Notarized copy of Gazette Notification for name change with unit holder self attestation 

    OR

  • Affidavit on Rs.100 Stamp Paper (Enclosed format) explaining the name change reason

  • For LIC MF - Rs.600 Stamp Paper

  • For Franklin - Rs.200 Stamp Paper

  • Self-attested Identity Proof with New Name

Important Notes:

  • The name on KYC, PAN, and Income Tax Department (ITD) records must match exactly for the change to be processed.
  • Ensure all documents are clearly legible and self-attested.
