RIs can complete the KYC by filling out the Individual_KYC_Form.pdf form attached below.
Form Filling Instructions:
Affix a passport-size photograph and sign across or below the photograph
Personal Details must be filled
(Investor name should exactly match ITD records)Proof of Address details must be filled
Ensure the address mentioned matches the address proof attached
If Correspondence Address (Section 3.2) is the same as Current/Permanent Address (Section 3.1), mention “Same as above”
If both addresses are different, address proof for both must be attached
Email ID and Mobile Number must be filled
Mention Date, Place, and Applicant Signature
Documents Required:
All the documents need to be self-attested

2. PAN Card Copy
3. E-Aadhar (Masked)
4. FATCA/CRS Declaration Form
Points to be Noted:
Offline KYC can be processed only when MFU KYC or Digi KYC has failed.
Digital KYC is always recommended as the first option.
